Post-Hospital Non-Medical Home Care in Chicago and the North Shore
Hospital-to-home support
The discharge date is set. Now everyday life has to work again.
Returning home after a hospital or rehabilitation stay can expose gaps that were not obvious before: meals, bathing, dressing, safe movement, laundry, transportation, and the need for someone to be present. Mafolux provides non-medical post-hospital assistance that supports daily life while the client follows discharge instructions from the medical team.
Post-hospital assistance may include
- Meal preparation and hydration reminders
- Personal care and dressing assistance
- Mobility assistance within the non-medical care plan
- Light housekeeping and laundry
- Transportation or accompaniment
- Medication reminders without administration
- Companionship and family respite
Plan before discharge whenever possible
Tell Mafolux the expected discharge date, address, daily routines, mobility needs, desired schedule, and tasks identified by the discharge team. Non-medical care does not replace clinical treatment, emergency services, or medical instructions.
How can care be paid for?
Mafolux Healthcare Services is Medicaid enrolled, accepts private pay, and works with clients using applicable long-term care insurance benefits or reimbursement. Medicaid services require applicable eligibility and authorization. Long-term care insurance coverage and reimbursement depend on the individual policy, benefit approval, documentation, and insurer.

Service availability depends on the person’s needs, location, schedule, authorization when applicable, and current caregiver availability.
